Welcome to the Secure Data System (SDS). The SDS has been developed to permit Company Security Officers (CSO) to submit Contractor security screening online using batch numbers provided by the Ministry/Commission Public Body (CPB)/Infrastructure Ontario (IO) Program Area Manager.

General Information and Instructions

The Contractor Security Screening Request and Verification form is to be completed by the Company Security Officer (CSO) and the Ministry/CPB/IO Program Area Contact. It contains information about the type of check requested and confirms whether verification of personal information and a reliability check has been completed for all individuals requiring a security screening check and clearance. Complete, print, sign and date the forms and send the original copies to the Ministry/CPB/IO Program Area Contact.

Delivery of Forms

The Company Security Officer (CSO) must forward the hardcopy Request and Verification form, accompanied by the applicant’s Consent and Declaration form (in a sealed envelope), to the Ministry/CPB/IO Program Area Contact via secure courier or by hand delivery. This is done in addition to submitting an application electronically using the on-line application.

Once verified, the Ministry/CPB/IO Program Area Contact will forward the completed Request and Verification forms along with the sealed Consent and Declaration forms to the Contractor Security Screening Unit (CSS), Supply Chain Ontario (SCO), Ministry of Government and Consumer Services (MGCS) for processing.

Important Note: Files will not be processed until signed and dated hard copy forms are received by the Contractor Security Screening Unit (CSS).
